Investigation Summary
Employee #1 is a truck driver who hauls wood chips. When he had completed loading his truck with chips from the chip storage bin at a sawmill, he pulled his truck forward. Using the ladder on the front of the truck, Employee #1 got on top of his load of chips and leveled it with his feet. He then pulled the tarp over the load. The tarp is situated so that it is normally maneuvered from the ground. Employee #1 went to the back of the trailer to use the ladder to get to the ground level. According to Employee #1, he put his foot on the first rung and as he shifted his weight from his right foot to his left foot, he lost his balance. Employee #1 fell backward 13 ft to the ground. He said he was not in a hurry and cannot explain why his foot slipped.
