Investigation Summary

Investigation Nr: 170195648
Event: 08/27/1990
Employee's eye injured by flying debris

At approximately 1:30 p.m. on August 27, 1990, Employee #1 was attempting to remove a bearing from the edger housing. He was attempting to loosen it with a punch and hammer when a piece broke off and flew back into his eye. A piece of metal buried itself in the edge of the cornea in his left eye. Employee #1 was not wearing safety glasses. He was transported to St. Mary's Hospital in Reno, NV the next day for surgical removal of the metal object. A cataract formed over his eye because of the injury and he will have to have surgery again to remove it. A serious citation was issued to the employer because Employee #1 was not wearing eye protection when exposed to flying debris.
