Investigation Summary
On July 17, 1990, Employee #1, a carpenter for Bill Madigan General Contractor, fell 8 ft from the top plate of a single story house. The employee fell when he stepped on a 2 by 4 that had been nailed across an area that was to be a hallway opening into a room. The 2 by 4 was nailed in place to support the end of a truss the employee was preparing to move. As he stepped on the 2 by 4, it gave way. The employee fractured his left wrist when he reached out to break his fall.
