Employee's hand injured by blender cutter blades

At approximately 11:30 a.m. on November 6, 1992, Employee #1, of Leprino Foods, was working in the quality control lab, grinding control samples of cheese using an 8-speed Oster blender, quart size jar. Apparently, the jar was not secured to the threaded container base unit. As the blender was turned on, Employee #1 held the jar with her right hand on top. The blender rocked the jar back and forth to make slurry. The blender jar slipped off the gasketed blender base. Employee #1 attempted to grab the bottom of the jar and her left hand contacted the rotating blender cutter blades. She sustained multiple lacerations of the left hand with skin loss and possible laceration of both digital arteries and nerves. She was hospitalized.
