Investigation Summary

Investigation Nr: 170171185
Event: 05/25/1995
Employee injured in fall through skylight

At approximately 7:00 p.m. on May 25, 1995, Employee #1, of Butterball Turkey Co., was working alone, installing evaporative cooler filter pads on the coolers located on the roof of a one-story building. Since the filters were not pre-cut, his work included cutting the filters to size from a roll of filter material. He was using a plastic-domed skylight fixture as a tabletop/work surface. Employee #1 was leaning on the skylight fixture, placing his weight on the skylight/dome area, when the skylight gave way. He fell through the opening to the concrete floor, sustaining multiple fractures. He was hospitalized.
