Employee dies when caught by asphalt grinder drum

At around 3:15 p.m. on August 23, 1993, Employee #1 and a coworker were preparing the Bartmill 300 asphalt grinder for work the following day. Employee #1, the operator, crawled next to the grinding drum and replaced several missing teeth. Then the coworker, the groundman (operator trainee on the Bartmill 300), brought the welding equipment to Employee #1 for replacement of three tooth pockets (holders). After welding one tooth pocket, Employee #1 told the coworker to start the engine and raise the drum so that he could weld the other two pockets. The coworker warned Employee #1 to get out first before he started the engine. Employee #1, however, assured the coworker that the drum was not engaged and to go ahead and start the engine. As instructed, the coworker started the engine. He heard Employee #1 scream and he shut off the engine. The grinding drum had rotated and caught Employee #1, who died from his injury.
