Employee's heel broken in fall when ladder slips

Employee #1 had been painting the outside of an 18 ft tall stucco shop building for the entire morning, moving his 24 ft aluminum extension ladder as he progressed. He was about 15 ft above the blacktop surface, painting the top of a column, when his ladder suddenly slipped out from under him and he fell to the asphalt. He sustained a severely fractured left heel. When Employee #1 had moved the ladder into this final position, he had checked its stability several times. The ladder was in good condition and both rubber safety feet were in place. The blacktop surface had a gradual slope away from the building.
