Investigation Summary
Employee #1, of GTE, was working from a baker board, a small work platform linemen stand/lean on, repairing a telephone cable. Employee #1 descended from the 24 ft level on the utility pole to the 12 ft level to check a connection. As he attempted to re-connect his safety belt/safety strap at the 12 ft level, he did not engage the connection of the strap hook to the belt's D ring. Thinking he did, the employee leaned back and fell 12 ft to the concrete surface. He sustained a fractured left arm and a dislocated right elbow.
