Investigation Summary
At approximately 2:45 p.m. on March 20, 2002, Employee #1 of Blue Diamond Materials was aiding the truck driver unloading two trailers of sand materials into the Grizzler Coldfeed Receptor. His foreman was with him as both of them were showing the truck driver the area where to unload the sand. The Grizzler Coldfeed Receptor is an area where sand were being unloaded and then conveyed to the repository for future use for asphalt processing. The truck driver, an independent contractor proceeded unloading the first load into the Grizzler and after finishing the first loading process which took approximately less than five minutes, he went back to the truck cab to start unloading the second trailer. The truck driver observed Employee #1 and his foreman operating the conveyor control panel by moving it back and forth. The conveyor control panel was located approximately fifty three inches from the right side of the truck's first trailer wheel. The truck driver started moving the truck forward very slowly and then he heard the foreman yelling back up. He backed up immediately and put the vehicle on brake. He went down and saw Employee #1 lying on his back on the ground. The foreman ran to the trailer and summoned help. The Paramedics arrived shortly and pronounced that Employee #1 had died.
