Employee injured when arm struck by wrench attached to shaft

At about 10:30 a.m. on February 8, 1991, Employee #1, a machine operator for Advance Paper Box Company, was assisting in the repair of a 1983 Wupa press paper box die cutting machine, model AR-50, #834, manufactured by Iberica AG of Barcelona, Spain. When electric power was restored to the machine, the shaft spun around and the wrench affixed to it struck Employee #1's left forearm, fracturing both bones. He was transported by Los Angeles Fire Department paramedics to the Daniel Freeway Hospital in Inglewood, CA. At the time of the accident, the machine's drive shaft had its guard off, the wrench was left affixed to the drive shaft, employees were too close to the machine, and a competent, knowledgeable person was not supervising and enforcing the employer's safety rules.
