Investigation Summary
At approximately 5:10 p.m. on April 14, 1992, Employee #1, age 17, was operating a forklift on dry, near-level pavement. An eyewitness stated that Employee #1 lost control of the vehicle after descending a slight incline. He apparently tried to jump clear of the machine as it began to overturn, but the ROPS pinned his head against the pavement. Employee #1 was killed. Coworkers called 911 and used a pallet jack to raise the fall protection rigging and free Employee #1. Paramedics arrived in about 10 minutes and a county medical examiner took charge of the body. Examination of the scene indicated that after descending the incline, the forklift drifted to the left, and skid marks showed that Employee #1 made a steering correction. About 16 ft further, the right wheels appeared to have lifted off the pavement. Left tire marks continued in a rightward circular path for another 15 feet, at which point the forklift overturned to the left. The lack of any slide marks on the pavement confirmed the eyewitness statement that the forklift had almost stopped when it rolled over.
