Employee's hip fractured in fall from roof

At 12:50 p.m. on April 8, 1991, Employee #1 was on a roof gathering loose plywood so it would not be hazardous to coworkers. He stepped on a piece of plywood that slipped out from under him, causing him to fall 22 ft onto the cement floor. Employee #1 sustained a fractured hip and was hospitalized.
