Employee's hand and arm crushed in bottling machine

At approximately 2:30 p.m. on May 28, 1991, the bottling line foreman at a winery was monitoring the production. He noticed that several bottles had fallen into the center of the filling machine. He was reaching into the revolving machinery when his left arm became entangled in the unguarded outer portion of the machine. His hand was bent backward and drawn into and through a 3/8 in. gap between the machine wall and the bottle filling pedestals, which at this point were moved downward by a pointed, cam-like lug mounted on the outermost portion of the die machine. Employee #1 attempted to reach the emergency STOP switch, but could not. His yells alerted a coworker, who was able to halt the machine just as Employee #1's elbow was about to be pulled into it. Employee #1 suffered a crushed left hand and arm up to his elbow, including a fractured wrist and separation of the radial and ulna at the wrist joint.
