Investigation Summary
At approximately 1:30 p.m. on December 6, 1991, Employee #1, a vineyard laborer, was on an elevated steel work platform that was attached to a three-wheeled forklift being operated by a coworker. The two employees had been assigned to place decorative lights on trees planted in a parking area. The slope of the paved surface varied from 10 1/2 degrees to 12 degrees at the point where the lift truck was parked. When the work was almost finished, the coworker tilted the mast back, then began to lower it from the 16 ft working height. The forklift immediately began to slowly tilt downhill. The tilting accelerated until the forklift, mast, and platform struck the ground, smashing Employee #1 inside the railed platform. Employee #1 sustained multiple fractures, including his left arm and leg, right hip, and right index finger. He was transported by ambulance to a hospital, where he stayed for more than four days.
