Investigation Summary

Investigation Nr: 170206296
Event: 09/16/1991
Employee crushed and killed as logs fall from truck

At a logging landing, a log truck had been loaded by a front end loader. The driver tossed a wrapper over the top of the load. Employee #1, the landing chaser, who was on the other side of the truck, caught the wrapper and threw it under the truck to the driver. At this point, a log above the bunk stakes fell off the truck on the side Employee #1 was on and hit him as he was running away from the truck. A second log landed on him as he lay on the ground, crushing him at the pelvic area, and killing him.
