Employee's finger amputated by swinging vise

Employee #1 has worked as a die maker for Factory Pipe for about two years. On September 3, 1991, he had finished welding a motorcycle exhaust pipe and was going to cut a hole in it using a hole saw attachment on a Powermate drill press. Employee #1 placed the exhaust pipe in a hand vise and secured it to the drill press table. Still wearing the cotton gloves he had had on during the welding operation, Employee #1 held the pipe with his left hand (thumb on top). As the saw started to cut through the pipe, the vise swung around and caught Employee #1's thumb between the hole saw and the pipe. Employee #1's thumb was amputated.
