Investigation Summary
At approximately 4:18 p.m. on November 12, 1990, Employee #1, a baler, and a coworker had finished work and were going to use an air hose in the shop to raise the air shocks on a pickup truck. They moved the truck to behind a double trailer rig that was parked in front of the office; the end of the trailer extended in front of the shop. Employee #1 was between the bed and bumper on the left rear side of the pickup truck, bending over to put air into the air inlet fitting. The truck driver came out of the office, looked down each side of his rig, entered the cab, and put his paperwork in a briefcase. He then backed up his rig to a power pole, running over Employee #1. Employee #1 was transported to Santa Rosa Community Hospital, where he died. He had worked for the company for 9 months.
