Investigation Summary
At approximately 8:40 p.m. on April 30, 1992, Employee #1 was operating a 14 in. chopper/extruder, serial #19409-577. The guard was not bolted down and was evidently not in use. Employee #1 was feeding a ribbon of hot melt adhesive into the extruder when his left hand was drawn into the rollers and three fingers were amputated. A serious violation of T8CCR 3203(a)(b) and a willful serious violation of T8CCR 4184(g) were cited.
