Investigation Summary
At 10:30 a.m. on February 23, 1996, Employee #1, an information specialist at the Northern California Cancer Center in Union City, was on break and engaged in an activity unrelated to her work. She and a coworker were carrying a very large empty cardboard box out to her car. Employee #1 was walking backward, carrying the front end of the box, when she tripped on the concrete step immediately after leaving the emergency exit door at the southeast corner of the building. She fell and fractured her right ankle.
