Investigation Summary

Investigation Nr: 170702906
Event: 02/25/1993
Employee crushes finger in conveyor belt/pulley drive

Employee #1 was banding rebar on a roll conveyor after it exited an alligator shear operated by the leadman. A section of the drive guard had been removed by maintenance employees. The problem had not been corrected and the unguarded unit was still in use. Employee #1 attempted to assist the rebar over an area of conveyor slippage when his gloved hand was pulled into the belt/pulley pinch point, crushing his right ring finger and lacerating his small finger. The leadman cut the belt after he was unable to free Employee #1 by reversing the conveyor direction. He was taken by car to Washington Hospital where the crushed finger was amputated to the first joint.
