Investigation Summary
At approximately 7:15 a.m. on December 10, 1991, Employee #1, an 18-year-old temporary employee with two months experience as a cartridge tech, was working at Costar Nuclepore, inserting the end of Lutrasil through the in-running side of a Rabofsky accordion pleating machine, model 88/100. Normally this machine is operated by a foot switch, with the main power switch on the side of the machine. It is also equipped with a presence-sensing device on the in-running side to prevent cycling in case workers are exposed to the pleating blades. Prior to the accident, two coworkers noticed Employee #1 wearing rubber gloves and using both hands to align the material into the machine. They also noticed that the pleater had material pullout which piled up and blocked the electric eye, causing the pleater to shut off. As the material uncovered the electric beam, the machine continued operations. Employee #1's fingers were struck by the knives of the pleating machine. She was transported to a local hospital by private vehicle, arriving at approximately 7:52 a.m. with fractured and lacerated fingers and an amputated fingertip. She was admitted to the hospital and underwent surgery. On March 3, 1992, Employee #1 had surgery for nonunion of the fractures. The accident was probably caused by inadequate guarding at the point of operation. Violations included T8CCR 6003(a)(1), general, and T8CCR 4184(b), serious.
