Investigation Summary
On April 24, 1991, Employee #1 was operating an automatically fed EMBA folder/gluer machine. Near the end of a stock run he saw a misaligned corrugated box and quickly reached into the feed rolls and belt area to straighten the stack. He inadvertently placed his left hand between the bottom rolls and the belt take-up roll. His hand became caught and was partly crushed and degloved on the back. He also suffered a crushed and fractured index finger and split and degloved left thumb. Employee #1 was taken by ambulance to Eden Hospital in Castro Valley, CA, for surgery and treatment. At the time this report was prepared, it was anticipated that his index finger would have to be amputated at the base. Employee #1 was not paying full attention to the job and misjudged the danger of the task. He had worked for Mark Container for 23 years.
