Investigation Summary
At approximately 11:00 a.m. on April 11, 1991, Employee #1 attempted to step from one section of scaffolding to another. The scaffolding section that he was stepping onto began tilting away from building, leaving a gap between the two sections. Employee #1 fell between the two sections of scaffolding to the ground, breaking both legs.
