Investigation Summary
At approximately 8:55 a.m. on November 1, 1990, Employee #1 was preparing to shoot stucco onto a building under construction. He was standing on the third plank out on a work platform at the second level, approximately 18 ft above the ground. As he took the nozzle from a coworker, the pressure forced him backward. Employee #1 leaned against the guardrail but it gave way and he fell to the ground. He suffered a stress fracture of his left heel.
