Investigation Summary
At approximately 2:30 p.m. on December 19, 1990, Employees #1 through #3 were masking and spray coating in a single-family house. They were using a Dynamite, model D-520 unit, with an ASM mid-400 gun to apply an oil-based primer when there was an explosion and fire. Employee #1 suffered severe burns on his hands and face, and was hospitalized until January 23, 1991. Employee #2 suffered severe burns over 70 percent of his body and died in the hospital on January 17, 1991. Employee #3 suffered a strained back and contusions on his right foot. He was treated at a local hospital and released. The furnace and heater pilot lights were not shut off, and either one could have been a source of ignition.
