Investigation Summary
At 3:45 p.m. on August 12, 1990, Employee #1 was replacing a standard light bulb in the southwest section of the east exhibit hall. The entire hall floor was thoroughly waxed with a protective coat to prevent damage from heavy foot traffic. Employee #1 obtained a single portable wooden ladder with a maximum height of 14 ft and a highest standing level of 10 ft 6 in. He placed the ladder, which did not have a non-slip base, against the wall without sufficiently securing it or having someone hold it. He ascended the ladder, changed the bulb, and, as he descended, slipped and fell from a height of 12 ft. He struck the ladder and the floor, suffering fractured ribs and resulting in pneumothorax. He was hospitalized for 7 days and, at the time this report was written, was expected to return to work in 3 months. The cause of the accident was Employee #1's failure to select a proper ladder to use on a slippery floor.
