Employee's fingers injured in table saw

At approximately 3:00 p.m. on August 18, 1990, Employee #1, a carpenter, was using a 10 in. table saw to cut a 6 in. diameter by 2 in. wide roll of fabric tape into two 1 in. wide pieces. He set the saw fence at 1 in. and held the tape with both hands as he began to cut. The action of the saw blade caused the tape to roll. Employee #1's right hand was pulled into the blade, amputating his right middle finger at the first knuckle. He also suffered lacerations to his right thumb and his right ring finger. This work was being done at the request of a coworker from the reprographics unit who, and with the foreman's permission.
