Employee's fingers crushed when caught in aniline roller

At approximately 8:20 p.m. on August 16, 1990, Employee #1, a corrugator stacker operator at the Inland Container plant, was assigned to prevent excessive ink buildup on the ink roller of the aniline machine. He was reaching in to clean the rollers with an SOS pad when the rollers caught the pad and pulled in his fingers and hand. He sustained crushing injuries to three fingers before he was able to jerk his hand free. Power to the aniline machine had not been disengaged, nor were positive means taken to prevent Employee #1's hand from being drawn into the rollers.
