Investigation Summary
On January 30, 1992, Employee #1, of Ross Stores, Inc., was performing scheduled maintenance on an elevated conveyor system. As instructed, Employee #1 had removed a chain drive cover and was lubricating the chain while it was moving. Employee #1 wiped excess lubricant from the chain with a paper shop rag that he had wrapped around his right index finger. His finger was pulled between the chain and sprocket and the fingertip was severed, excluding a small portion of the nail. Employee #1 was taken by ambulance to Washington Hospital for treatment.
