Investigation Summary
At approximately 10:30 a.m. on October 10, 1996, Employee #1 was using a radial arm saw to cut pieces of wood to length, manually holding the wood on the saw table with his gloved hands. He had finished a cut and was returning the saw to its back position. Although the saw was switched off, the blade was still spinning and it caught the glove on Employee #1's right hand. amputating one of his fingers. Employee #1 was transported to the hospital, where his finger was reattached.
