Investigation Summary
At approximately 9:45 a.m. on September 27, 1996, Employee #1 was placing items on a conveyor belt. She stooped to retrieve some boxes that had fallen to the floor, placing one hand on the slowly moving belt. Her palm sustained lacerations when the belt nipped it against the edge of a support plate.
