Investigation Summary
Employee #1, a plumber for Can Am Plumbing, was completing the installation of roof jacks around the vents of the 5:12 sloped roof of a single-family house under construction. He had previously installed the same roof jacks on at least four similar houses in this development tract. Roof jacks are installed around the vents before roofers start laying paper. Employee #1 started his shift at 7:00 a.m. and by approximately 8:30 a.m., he had only one more vent to go. He was approaching the last vent near the edge of the roof when he slipped and fell about 20 ft to the ground. Employee #1 sustained multiple fractures in both heels, for which he was hospitalized. At the time of the accident, the roof was still moist and slick. Employee #1 had not been provided any fall protection.
