Employee's ribs fractured in fall when ladder slips

Employee #1 was using a 12 ft ladder on a concrete floor to inspect a bin above some coffee grinders. The ladder slipped out and he fell approximately 8 ft, landing on top of the ladder. Employee #1 suffered a hairline fractures of his ribs. The ladder was a section of an extension ladder and did not have non-skid bases.
