Investigation Summary
At 10:30 a.m. on August 19, 1996, Employee #1, who was part of a crew installing new roofing on an existing apartment building, climbed onto the back of a truck used for hauling materials to the job site to remove a tarpaulin. As he pulled on the tarp, it became caught on a piece of metal and then suddenly broke free. Employee #1 lost his balance and fell between 9 and 10 ft to the ground. He was treated at the hospital and released the same day.
