Employee injured in fall from roof

At approximately 4:10 p.m. on December 7, 1995, Employee #1, of Security Roofing Systems, was sent to the roof of a building to cover stripped areas with Visaqueen plastic sheets as protection from the weather. He was laying a sheet near the edge of the roof when he slipped on the plastic and fell 18 ft onto a wooden deck. Employee #1 suffered a fractured jaw, severe bruising on one side of his body, and bleeding from one ear. He was hospitalized for treatment and observation.
