Employee injured in fall from ladder

At approximately 2:30 p.m. on November 16, 1995, Employee #1, of Sampac Enterprises, was using a stepladder to gain access to a box on an overhead storage shelf. He was reaching for the box when he lost his balance and fell to the floor. He sustained injuries to his head and back and was transported to the hospital, where he was treated and released.
