Investigation Summary
At approximately 2:00 p.m. on February 15, 1991, Employee #1, of Howard Hughes Medical Institute, was removing four 10-liter bottles from an autoclave. She waited five minutes to allow for the release of steam, removing each of the bottles separately. As she was removing the fourth bottle, it broke, and hot, 200 degrees F water deluged her legs. She screamed to a nearby coworker for help. Employee #1 suffered deep, partial-thickness burns, requiring 3 weeks of hospitalization. She stated that she had never received specific training on how to handle large, 10-liter bottles. Employee #1 further stated that the general procedure she was taught was not practical for this size of bottle. In light of the accident, the employer initiated a protocol to use 8-liter bottles or smaller, and provided a metal cooling pan large enough to contain bottled liquids.
