Investigation Summary
At approximately 2:15 p.m. on January 9, 1991, Employee #1, of Linford Air and Refrigeration Company, mounted the platform of a scissor lift and was raised about 21 ft to a 5 ft diameter air duct. He climbed atop the duct and was stepping onto an adjacent catwalk when he slipped through the 18 in. gap between the catwalk and the ductwork and fell to the floor. No fall protection was in use.
