Investigation Summary
Employee #1 was going to paint the top screen area of a chimney and used a ladder to climb to the roof's edge. He had just climbed to that level while carrying a 5-gallon paint container in his left hand, leaving only his right hand to grip the ladder and to help him ascend. When he got to the ladder's top rung at the roof's edge, he stepped from the ladder onto the roof, which was still wet from the morning dew. When he let go of the ladder, his foot slipped and he fell off the roof. Employee #1 was hospitalized.
