Employee's finger nearly severed by machine shear point

At approximately 11:00 a.m. on October 2, 1991, Employee #1 was cleaning the dough hopper of a dividing machine on a production line making loaves of bread. To reach the parts to be cleaned, Employee #1 opened the door on the machine which normally would activate a microswitch and turn off the machine's power. A coworker, however, had deactivated the switch. Consequently, when the machine operator requested more dough, the dough divider blade moved and caught Employee #1's right hand in a shear point. His fingers were almost severed. Employee #1 was hospitalized for more than one week.
