Investigation Summary
At approximately 9:20 a.m. on December 18, 1990, Employee #1, of Plumbing 'n' Things, was using a forklift truck to unload packaged Jacuzzi tubs. The tubs were not palletized and could not be lifted by the truck's forks. As a result, Employee #1 walked onto the elevated forks, on which a pallet rested, to try and maneuver a package onto the forklift. As he pulled on the wood framing, it came away in his hand and he stumbled backward. Employee #1 fell about 9 ft from the unprotected edge of the forklift platform to the ground. He was treated at Sequoia Hospital for a broken wrist.
