Investigation Summary
Employee #1 was reaching under a truck chassis to grab the edge of the lift gate, thinking this was the proper way to open it flat. Simultaneously, he bumped the control linkage underneath, activating the power lift. His hand was still resting on the edge of the lift gate and four of his fingers were crushed between the gate edge and the truck's chassis.
