Investigation Summary

Investigation Nr: 170169205
Event: 02/17/1991
Employee injured in fall on wet floor

At approximately 12:00 noon on February 17, 1991, Employee #1 was passing out medication in a patient's room at the Beverly Manor Convalescent Hospital when she slipped on the wet floor. She fell on her left side, striking her knee, arm, shoulder, and neck. Conflicting information made it impossible to confirm whether a wet floor sign was in place. An informational memorandum was issued to the employer noting the requirement for a wet floor sign when the floors were being washed.
