Investigation Summary
On January 23, 1991, Employee #1, a carpenter, was working on a nearly flat warehouse roof. There was a hole in the roof through which ran an electric cord; it was covered by a 4 ft by 8 ft wooden roof panel that was neither marked nor secured. Employee #1 fell 21 ft through the hole onto concrete and suffered a fractured skull. At the time this report was written, it was expected that he would be disabled for months. No fall protection was provided, although it is needed for this type of building erection. A citation was issued for not marking or securing the roof opening. The foreman was aware of the temporary roof opening.
