Employee's finger injured in pipe and threader

At approximately 9:55 p.m. on December 14, 1990, Employee #1 was setting up a Collins pipe and threader, model A60. He was raising the spring-loaded stand to the work position when a part of the stand broke, possibly the pin holding the legs together. When the stand broke, something struck his hands and amputated the tip of his finger. No safety violations were observed that resulted in this accident. The machine was less than 1 year old and had no known defects. The accident occurred without warning and may have been due to a design defect in the manufacturing of the threader and in the particular pins holding the stand legs together.
