Employee's back broken in fall from elevated platform

Employee #1 worked for a personnel leasing agency and was under the direct supervision and control of a firm that manufactured steel building supplies. He climbed a ladder to the top of a 12 ft high storage platform to arrange some boxes. Employee #1 fell from the platform and suffered a broken back. The platform was not equipped with any guardrails.
