Investigation Summary
On September 30, 1991, a crane of Koehring Cranes and Excavators, model LRT 200D, was distributing plates of steel around the perimeter of a fuel tank under construction. The ground around the tank perimeter on which the crane was working was smooth and angled down from the tank base between 5 and 10 degrees. After walking two plates to the desired spot, the crane stopped. Employee #1, in the cab, noticed that the crane turret began turning to the right away from the tank. As the load moved to the downhill side, he attempted to stop the rotation by actuating the swing lever to drive the turret in the opposite direction. When this did not stop the right rotation of the turret, Employee #1 set the load on the ground. When rotation continued, he stepped on the swing brake. Employee #1 stated that the swing brake pedal went all the way to the floor and the rotation continued. The crane began tipping away from the fuel tank. Employee #1 jumped out of the cab and landed on both hands as he hit the ground, breaking both wrists. The crane apparently tipped over because it was unsafely loaded while on rubber on an unlevel surface. An allegation that the crane was mechanically defective was not substantiated by the certifying agency that inspected the crane after the accident. The swing brake was functional and not worn, the agency reported.
