Employee killed in fall from open-sided floor

At approximately 2:00 p.m. on October 25, 1991, Employee #1 was helping a journeyman carpenter cut an approximately 107 in. long 4 by 12 wooden header. Employee #1 was standing 3 ft from an open-sided floor, holding a wooden 4 by 4 that was approximately 82 in. long. He stumbled backwards and fell 27 ft to the ground. Employee #1 sustained multiple head injuries and died. There were no witnesses to the accident itself. When the carpenter, who had his back to Employee #1, felt the header move, he turned and saw Employee #1 fall over the edge.
