Investigation Summary
An employee was working at the cyclone deck of an electric power generation plant. He was checking the six cyclones. Through an angle viewing port, the employee saw that one of the cyclones was clogged. He removed the viewing port cover and attempted, without success, to rod out the blockage. The air cannon discharged, and hot fly ash blew out of the viewing port and hit the employee in the face. He was hospitalized for burn treatment.
