Employee's finger cut by cut-off knife

At approximately 12:30 a.m. on February 6, 1992, Employee #1, of Alza Corporation, was working as an inspector in the Circle 6 Pouching Line, which produces transdermal nicotine patches. The lead person stopped the pouching line due to a jam in the front knife. While the jam was being cleared and before the machinery was started, Employee #1 saw a piece of nicotine patch stuck on the rear cut-off knife. She tried to remove it to avoid a jam when the machinery started. Her right ring finger was caught and cut by the unguarded cut-off knife.
